Most of what you see is also from neglect as far as hygiene or a lack of knowledge of how to deal with it (maybe even a lack of tools to do so). That is why his nails are so long and thick. He may not be able to cut them at all at this point without medical assistance. Life in a third world country isn't all that glamorous.
